Our Services

Community Health Nursing
Initial assessment: Our experienced nursing team will conduct an initial assessment to identify our client’s healthcare needs and develop a care plan. This may involve reviewing medical records, conducting physical assessments and consulting with other healthcare professionals
Coordination of Care
We work with our clients, their families and healthcare professionals to coordinate your care. This may involve arranging appointments, monitoring medication, coordinating services such as physiotherapy and occupational therapy
Health Monitoring
Our trained nursing team monitors our client’s health status on an ongoing basis which involves measuring vital signs such as temperature, pulse rate, blood pressure and blood sugar. Monitoring medication efficacy and wound healing
Assistance with activities of daily living (ADLs)
ADLs are a part of Nursing Team’s core services that includes assistance with bathing, dressing, toileting, as well as providing support for mobility and transfers. So we ensure that our clients are well taken care of within home premises as well as outside
Education:
Prevention is better than cure. Awareness is a prerequisite for independent living. We educate our clients on critical topics such as wound care, medication management and disease prevention
Emotional Support
Our nurses and carers are adequately trained to provide emotional support to our clients and their families during their difficult times
Documentation and Reporting
Our nursing and carer team always maintain accurate and up-to-date documentation of care provided including progress notes, medication administration records and incident reports. Any changes in our client’s health status is promptly reported to healthcare professionals as appropriate
